Job Purpose

Job Duties and Responsibilities

• Conduct initial and ongoing assessments of patients' health conditions.
• Develop and implement individualized care plans based on patient needs and physician instructions.
• Administer medications, monitor vital signs, and perform necessary medical procedures.
• Educate patients and their families on managing health conditions and post-treatment care.
• Coordinate with physicians, therapists, and other health care professionals to ensure comprehensive care.
• Document all patient interactions, treatments, and progress accurately.
• Respond promptly to emergencies and changes in patient conditions.
• Maintain patient confidentiality and uphold ethical standards.

Required Qualifications

• Registered Nurse (RN) license in good standing.
• Proven experience in nursing, preferably in home health care or community nursing.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Valid driver's license and reliable transportation.

Preferred Qualifications

• Certification in home health care or related specialty.
• Knowledge of chronic disease management.
• Experience with electronic medical records (EMR) systems.
• CPR and First Aid certification.
