Job Purpose
To provide high-quality nursing care to patients in their homes, promoting recovery, managing health conditions, and supporting patients and their families in a compassionate and professional manner.
Job Duties and Responsibilities
Conduct initial and ongoing assessments of patients' health conditions.
Develop and implement individualized care plans based on patient needs and physician instructions.
Administer medications, monitor vital signs, and perform necessary medical procedures.
Educate patients and their families on managing health conditions and post-treatment care.
Coordinate with physicians, therapists, and other health care professionals to ensure comprehensive care.
Document all patient interactions, treatments, and progress accurately.
Respond promptly to emergencies and changes in patient conditions.
Maintain patient confidentiality and uphold ethical standards.
Required Qualifications
Registered Nurse (RN) license in good standing.
Proven experience in nursing, preferably in home health care or community nursing.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills.
Ability to work independently and manage time effectively.
Valid driver's license and reliable transportation.
Preferred Qualifications
Certification in home health care or related specialty.
Knowledge of chronic disease management.
Experience with electronic medical records (EMR) systems.
CPR and First Aid certification.
